Our office is currently accepting new patients. To schedule your first appointment, please give our office a call at 479-521-4822. A member of our friendly staff will work with you to reserve a time for you. If for some reason you are unable to keep your appointment time, please contact us as soon as possible to reschedule.

Broken Appointment Policy

Life is full of unexpected moments, so we understand last-minute changes to your schedule may be unavoidable. We will accommodate those changes as best we can. Please understand, when you make an appointment with us, we are setting aside that time for you — and to do our best work. Each appointment is for only one patient — we do not overbook and expect some of our patients to not show up for their appointments. A broken appointment without adequate notice results in wasted time for us, adding to the cost of providing care for all our patients.

Fee Structure

To be fair, if you have an unavoidable conflict and cannot give a 24-hour notice to change the appointment, there will be no charge the first time this happens. The second subsequent cancellation for the same appointment will incur a fee of 1/3 the scheduled amount of treatment. A third subsequent cancellation of the same appointment will require prepayment of the entire fee prior to rescheduling and the fee forfeited if the appointment is not kept.

Giving us 24-hour notice usually allows us to fill in the appointment — and there will be no broken appointment fee. We work with each patient to schedule times that are best for you. Patients who habitually reschedule for inadequate reason will be referred to an office which does not work by appointment or which can accommodate an unpredictable schedule.

Our Financial Policy

Our fees are determined by the materials we use and the quality care your receive. To help you calculate your costs and maximize your insurance, we will assist you with your benefit eligibility before your treatment. We are sensitive to your financial circumstances and will do everything possible to help you achieve good oral health. Regardless of your insurance coverage, you are responsible for payment.

Payment Options

We accept the following forms of payment: Cash, Check, Visa, Mastercard, American Express, and Discover. We offer a 5% discount for all treatment over $2000 paid in cash or check. This discount is reserved for cash patients without insurance benefits.

Payment for services is due at the time services are rendered unless prior arrangements have been made. Checks that are returned to our office from your financial institution are subject to a $25 returned check fee. This fee covers the processing fees that are charged to our office.

Insurance

Your insurance policy is an agreement between you and the insurance company and you are directly responsible for all charges. Your co-payment is due at the time of service. Our staff will submit any necessary claims to help you receive the maximum benefits available to you. Dental insurance plans typically do not cover 100% of your treatment. Combined with the delay in receiving payment from insurance companies, we ask that you pay your deductible and portion of the charges the day of your appointment.

Non-Insurance

Payment for services are expected the day the services are rendered. After determination of the required treatment, we will work with you to sequence treatment and provide payment options that best suit you and that also allow us to provide the best possible care.
